As a registered private hospital, we facilitate claims for both Gold and Silver Agrahara cardholders. Our dedicated insurance counter handles the paperwork so you can focus on recovery.

Hospitalization

Coverage for room charges, nursing care, and in-patient treatment costs depending on your entitlement limits.

Surgeries & Tests

Claims for major surgeries, cardiac procedures (Bypass/Stent), and expensive diagnostic tests like MRI/CT Scans.

Spectacles & Dental

Reimbursement support for eye care (spectacles/lenses) and specialized dental treatments available at our units.

Required Documents

To ensure a smooth admission and claim process, please ensure you bring the following documents when visiting the admission counter.

Agrahara Application Form

Duly filled and certified by the Head of your Department.

Valid Identification

Your National Identity Card (NIC) and Official Service ID Card.

Salary Slip

Most recent Salary Slip to verify Agrahara contribution deduction.

Dependent Proof (If applicable)

Marriage certificate (for spouse) or Birth certificates (for children).
